Transitional Independent Living

Transitional Independent Living is a short term, supportive housing program designed to help people who are homeless transition to self-sufficient living. These facilities are staffed 24-hours daily with caring, nurturing people who love what they do. There are spaces for community shared activities, bedrooms and bathrooms. Three (3) meals are provided daily, along with snacks and residents are responsible to clean-up their living space and in help keeping the community areas clean. Residents are required to meet with their case managers, go to clinic appointments and work toward improvement.

Program Includes:
  • Three meals daily

  • Medication administration
  • Offering mental health treatment in an individual or group setting
  • Transportation

Medical Housing

This program was created for homeless individuals discharged from a medical facility who need additional care in an outpatient setting but do not have housing to support them during this recovery phase. Often those with medical conditions remain in a hospital setting, not based on medical necessity but instead on not having a safe place to go. For the homeless and displaced people, this offers an environment more conducive to recovery and encourages independence, whenever possible.

  • Basic skills/activities
  • Coordination with care providers for on-going home care onsite

Community Based Living Arrangement (CBLA) Housing Program

The State of Nevada licenses CBLA homes. The CBLA Housing program provides person-centered services to include training and habilitation to persons with Serious Mental Illness (SMI). The CBLA Housing program is designed and coordinated to assist clients diagnosed with SMI to maximize their independence in the community. CBLA staff are on-site 24/7 to provide supervision and training to this vulnerable population. Staff work one on one with clients to provide individualized training and support such as medication management, management of mental health symptoms, safety and security, meal planning/preparation, and other activities of daily living. All CBLA staff complete initial and ongoing training mandated by the State of Nevada to maintain CBLA licensure.

  • Assistance with activities of daily living (ADL’s)

When any of our residents are dealing with substance use problems, we can offer them an array of services and programs to assist them with making healthier choices and improving their wellness through our behavioral services.

Offering mental health treatment in an individual or group setting, our organization offers one-on-one sessions utilizing telehealth or in-person with trained clinicians available to assist our residents by meeting their treatment goals. We offer groups as well as on-site, focused on the needs of those we serve.

We understand that not all our residents may have access to transportation. At New Hope Placement we offer transportation for doctor appointments and medical services.
